Any withdrawal of this consent must be submitted via email to: protecciondatos@gironafc.cat.

I expressly authorize Girona FC SAD to transfer the data contained in this form (those corresponding to the father / mother / guardian) to the Club's collaborator and sponsor, MGC Mútua (MGC Insurance, Mutua de Seguros y Reaseguros a Prima Fija), in order to offer its extensive insurance coverage through any usual communication channel, including electronic or telephone, guaranteeing the rights of access, rectification and deletion, among others, which can be exercised by contacting the registered office of the Club or that of MGC Mútua (Avinguda Montilivi, 141 – 17003 – Girona and Carrer Tuset, 5-11, baixos – 08006- BARCELONA, respectively) or to the email address campus@gironafc.cat, in the case of the Club, and dpd@mgc.es, in the case of MGC Mútua

Safeguarding Statement

Likewise, GIRONA FUTBOL CLUB, S.A.D. has informed us that it has an internal complaints channel: proteccionmenores@gironafc.cat . The Club guarantees the appropriate management of complaints and reports made, guaranteeing their confidentiality. The sports facilities where the campuses will be held are a video-surveilled area, so the image of our minor child may be recorded for identification, control and security.

If during the period of participation of my son/daughter at the Girona FC Campus, they present any of these illnesses, I undertake to communicate this to the coordinator of the corresponding headquarters in advance of the activity.
