Authorization for participation and healthcare

By registering for the Club’s esplai, you authorize and/or declare:

• That my son/daughter or minor in my care may participate in all activities organized by the esplai at the Club Tennis Sabadell sports facilities.

• That my son/daughter or minor in my care may swim in the pools and take part in water activities.

• That my son/daughter or minor in my care may be treated by a doctor in case of accident or illness.

• That the esplai staff may provide first aid if necessary.

• I declare that my son/daughter or minor in my care is up to date with the vaccinations corresponding to their age.

I declare that if I have any intolerance, allergy, chronic or recurrent illness, physical or mental disability, need to take any medication or any other issue that I consider necessary and important for the club to be aware of, I will communicate this in writing and provide all the corresponding health or medical information.

I authorize my child, guardian, to be accompanied by the playground staff using a private vehicle for non-urgent or specialized travel, in the event of needing medical attention, using the relevant safety measures. (voluntary option)

I expressly authorize the image of my child or legal ward to be captured, individually or in group settings, during activities organized by Club Tennis Sabadell, and used for informational or promotional purposes on the Club’s communication channels (website, social media, internal publications, etc.) and in external media. If this authorization is not granted, the Club will ensure that the minor does not appear identifiable in photos or videos, taking the necessary measures to protect their image and identity.
